SUSTENTATION THREADS (Russian threads)

The passage of time leads to the formation of expression lines caused specifically by the lower production of hyaluronic acid, collagen and elastin, all essential factors and nutrient elements from the dermis of the skin.

Elasticity is also decreased; leading to flaccidity that is accentuated by the force of gravity. When flaccidity is mild may be cosmetically annoying, but does not require plastic surgery, because it would produce an overcorrection leaving a face unnaturally rigid and artificial.

9 years ago, two Russian doctors, Marlen and George Sulamanidze invented and patented suture material threads (polypropylene suture used for surgical repair of wounds) with spikes that when placed under the skin would get hooked on it and when tensed they would lift the saggy tissue fixing the flaccidity of the area to be treated.

It has been many years after the invention and as such has been studied, observed and criticized by their peers, up to being accepted and improved. There are many detractors of this technique, especially doctors prone to a more radical surgery by a more aggressive technique, with the risk of complications and not indicated for mild to medium cases.

Today we have lift threads of different brands. There are even some made by amateurs at home than with a scalpel to make spikes on a polypropylene thread number 0 and sterilizing it  before use, which has led to a denigration of the technique.
The quality of the threads used and the professional experience of the one who is applying them are the essential two factors responsible for a good result. It is a guarantee for the physician and patient to be based on verified scientific studies carried out by the manufacturing companies. There should only be used materials that have quality certification.

In my experience I have had very good results with threads that are applied under local anesthesia, without risk, as an outpatient procedure, leaving almost no bruising and allowing the patient to return to their activities the following day.

The technique is indicated for patients with mild to moderate hypotonic tissue (flaccid) and to give some volume. It is recommended for cases of mild to medium facial flaccidity to produce an increase of volume at the cheekbones, but also for patients with fear of the conventional surgery and for those with contraindications to general anesthesia. Finally, areas where it is recommended and approved its use are the supraciliar area (above the eyebrows), jaw and neck.

The passing of the years produce collapse of the skin (ptosis) and highlight of facial wrinkles because of the flaccidity, giving the typical bull dog-faced appearance, or the accentuation of the nasogenian folds or puppet face. This is a minimally invasive procedure done with local anesthesia, painless, does not require hospitalization, nor postoperative recovery, achieving immediate excellent results when the entry of the thread is done correctly.
